Diagnostic Assessment

The Diagnostic Assessment (“Assessment”) is a clinical evaluation provided by a licensed practitioner. The Assessment can be conducted face-to-face or via telehealth. The Assessment is an evaluation of a client’s mental health, developmental disability, or substance abuse condition and results in the issuance of a Diagnostic Assessment report with a recommendation regarding whether the recipient meets enrollment criteria.  The outcome of mental health assessment is to determine the need for care and recommend appropriate service/treatment or the need for further assessment.
